Meet the Fox Chase team

Administrator: Jean A. Raiche

Jean Raiche is a licensed Nursing Home Administrator with 20 years experience in the health care industry. She has had experience in home health care, community service, rehabilitation and nursing home administration. She has held administrator positions in five states and served as regional manager for fifteen states. Jean holds a Masters Degree in Health Care Administration from Cardinal Stritch College, Milwaukee, a Masters Degree in Education from University of P.E.I., Canada, and a Doctorate Degree in Counselor Education from the University of Maine.

Nurse Liasion: Mary Apple Castillo, RN

Mary Apple Castillo, RN has returned to Fox Chase as our Nurse Liasion. Mary Apple, during the past nine years, has a wide range of nursing experience in both sub-acute, hospital and long term care settings. Her favourite past-time currently is spending time with Zach, her son.

Director of Nursing Services: Yvelyne St-Fleur

Yvelyne St-Fleur is a Registered Nurse who has been in long term care for 10 years. She came to Fox Chase with much experience in Cardiac and critical care nursing. She is certified in MDS, ACTs and most recently comlpeted her Certification as a Director of Nursing. She enjoys sharing her knowledge and experience with others.

Rehabilitation Director: Mark Pellas

Mark Pellas is a licensed physical therapist with more than 9 years of experience. He has been head of the rehab program at Fox Chase for the last 8 years. Prior to joining Fox Chase, Pellas was rehab director for "Nella's (PELLA's) Inc." in Elkins, WV.

Director of Social Services: Joel Rogers

Joel Rogers is a licensed, professional social worker dedicated to the welfare of each resident at Fox Chase. He is well-versed in patient rights, advanced directives, living wills and other issues of importance to residents and their families. Joel holds a master’s degree in social work and has served Fox Chase's residents and families for over ten years.
